رشد جسمانی در بیماران مبتلا به بتا تالاسمی ماژور در مشهد

<p align="right"> چکید ه </p><p align="right"> <strong><i> سابقه و هدف </i></strong></p><p align="right"> <strong> مبتلایان به تالاسمی ماژور، باید از زمان تشخیص بیماری تا آخر عمر هر 3 الی 4 هفته یک بار خون دریافت نمایند. اگر چه تزریق خون، سبب از بین رفتن علایم حاد بیماری میگردد، ولی عقب افتادگی شدید رشد جسمانی ناشی از اضافه بار آهن را در دهه دوم عمر به جا میگذارد. </strong></p><p align="right"> <strong><i> مواد و روشها </i></strong></p><p align="right"> <strong> پژوهش حاضر یک مطالعه مورد ـ شاهدی بوده که به روش مقطعی به بررسی رشد جسمانی در مبتلایان به تالاسمی ماژور پرداخته است. گروه مورد شامل 100 بیمار 2 تا 18 ساله مبتلا به تالاسمی ماژور و گروه شاهد مشتمل بر 340 دختر و پسر سالم در همان رده سنی بودند. </strong></p><p align="right"> <strong><i> یافتهها </i></strong></p><p align="right"> <strong> در گروه مورد، میانگین و انحراف معیار میزان هموگلوبین و فریتین، به ترتیب 5/1 </strong><strong>± </strong><strong>5/8 گرم در دسیلیتر و 525 </strong><strong>± </strong><strong>2183 نانوگرم در میلیلیتر بود. یافتههای ناشی از وزن، قد ایستاده و نشسته در گروه مورد نشان داد که وزن 9/48%، قد ایستاده 4/53% و قد نشسته 3/73% دختران و وزن 7/52%، قد ایستاده 4/56% و قد نشسته 5/74% پسران، زیر صدک پنج منحنی رشد قرار داشت. نتایج ناشی از مقایسه قد و وزن گروه مورد و شاهد نشان داد که با افزایش سن، اختلاف معناداری در اکثر گروهها دیده میشود. </strong></p><p align="right"> <strong><i> نتیجه گیری </i></strong></p><p align="right"> <strong> نارسایی رشد، یافتهای طبیعی در مبتلایان به بتا تالاسمی ماژور است. اصلیترین علل نارسایی رشد در این گونه افراد شامل آنمی، هیپوکسی مزمن و اضافه بار آهن میباشد. </strong></p><p align="right"> </p>
<p> <strong> Abstract </strong><strong></strong></p><p> <strong><i>Background and Objectives</i></strong></p><p> Thalassemia represents a group of recessively inherited hemoglobin disorders and is characterized by reduced synthesis of beta globin chain. The homozygous state results in severe anemia which needs regular transfusion every 3-4 weeks. Frequent blood transfusion can lead to iron overload which may result in growth retardation in thalassemic patients.</p><p> </p><p> <strong><i>Materials and Methods</i></strong></p><p> In this descriptive-cross sectional study, we measured weight and height of 100 patients with beta thalassemia major within the age range of 2-18 years old who were under follow up in Dr. Cheikh Hospital. The results were then compared with the control group (340 healthy children). </p><p> </p><p> <strong><i>Results</i></strong></p><p> The mean and standard deviation values of hemoglobin and ferritin were 8.5 ± 1.5 g/dl and 2183 ± 525 ng, respectively. Short stature was seen in 53.4% of girls and 56.4% of boys. Short thrunk was reported in 73.3% of girls and 74.5% of boys. Underweight was reported in 48.9% of girls and 52.7% of boys. </p><p> </p><p> <strong><i>Conclusions</i></strong></p><p> Growth retardation is common in thalassemic patients. Therefore, height and weight should be routinely and frequently assessed and in the case of growth retardation the cause should be investigated. Common causes of growth retardation are chronic anemia, hypoxia, and iron overload. </p><p> </p>
