بررسی عوامل خطر هپاتیت B در اهداکنندگان خون استان اصفهان در سال 1383

<p> چکید ه </p><p> <strong><i>سابقه و هدف </i></strong></p><p> <strong> هپاتیت B از بیماریهای خطرناک ومسری عفونی بامیزان مرگ ومیر و ازکارافتادگی بالا محسوب میشود. آمار آن درمناطق مختلف بسته به وفور رفتارهای پرخطر وسطح آگاهی مردم متفاوت است. لذا این مطالعه باهدف شناسایی دقیق عوامل خطرهپاتیت B با استفاده از گروه شاهد در اهداکنندگان خون اصفهان انجام گردید. </strong></p><p> <strong> </strong><strong><i>مواد و روشها </i></strong></p><p> <strong> در یک مطالعه تحلیلی، 300 اهداکننده شامل 39 فرد </strong><strong>HBsAg </strong><strong>مثبت و261 فرد </strong><strong>HBsAg </strong><strong>منفی به روش نمونهگیری غیراحتمالی آسان از بین مراجعهکنندگان به مرکز انتقال خون اصفهان جهت اهدای خون در تابستان 1383 وارد مطالعه شدند. جهت تعیین افراد </strong><strong>HBsAg </strong><strong>مثبت از روش الیزا استفاده گردید. سپس از طریق تکمیل پرسشنامه استاندارد شده از نظر روایی و پایایی عوامل خطر بیماری در مورد تمامی نمونهها و تعیین فراوانی نسبی هر یک از عوامل خطر در دو گروه مورد و شاهد با استفاده از نرمافزار 10 </strong><strong>SPSS </strong><strong>و تست آماری کایدو </strong><strong>(Chi-square) </strong><strong>مقایسه انجام شد و 05/0 </strong><strong>p< </strong><strong>معنی دار تلقی شد. </strong></p><p> <strong><i> یافتهها </i></strong></p><p> <strong> فراوانی سابقه عمل جراحی (003/0 </strong><strong>p= </strong><strong>و 61%)، سابقه استفاده از تیغ مشترک (049/0 </strong><strong>p= </strong><strong>و 8%)، سابقه زردی مادر (001/0 </strong><strong>p= </strong><strong>و 10%)، وجود فرد </strong><strong>HBsAg </strong><strong>مثبت (001/0 </strong><strong>p= </strong><strong>و 31%) و معتاد تزریقی در خانواده (011/0 </strong><strong>p= </strong><strong>و 10%) بین دو گروه اختلاف معنیداری را نشان داد. در دو گروه هیچ اهداکنندهای با سابقه دیالیز، سابقه قمه زدن، سابقه فرورفتن اتفاقی سرنگ آلوده به بدن و سابقه مصرف مواد مخدر تزریقی وجود نداشت. </strong></p><p> <strong><i> نتیجه گیری </i></strong></p><p> <strong> با توجه به این که فراوانی نسبی سابقه عمل جراحی در دو گروه مورد و شاهد از نظر آماری معنیدار بود، توجه به بهداشت بیمارستانها و اتاقهای عمل ضروری بهنظر میرسد. وجود فرد HBsAg مثبت و معتاد تزریقی در خانواده بین دو گروه اختلاف معنیداری داشت، آموزش و واکسیناسیون در گروههای پرخطر شامل خانواده فرد مبتلا و خانواده افراد معتاد در کاهش شیوع بیماری میتواند مفید واقع شود. فراوانی نسبی سابقه استفاده ازتیغ مشترک و زردی مادرکه معمولاً درکشورهای با شیوع بالا ازعوامل خطر عمده محسوب میگردد، در این دو گروه از نظر آماری معنیدار بود که بر توجه مسؤولین به آموزش جامعه از راههای سرایت بیماری و غربالگری خانمهای باردار تأکید دارد. </strong></p><strong><i>کلمات کلیدی: </i>هپاتیت B ، عوامل خطر، اهداکنندگان خون، اصفهان </strong>
<p> <strong> Abstract </strong></p><p> <strong> </strong></p><p> <strong><i>Background and Objectives</i></strong></p><p> Viral hepatitis B is a dangerous disease with high mortality and morbidity rate in the world. It has been proved that its prevalence in different areas depends on risk behaviors and people’s awareness level. This paper was prepared to detect the risk factors of hepatitis B in blood donors in Isfahan province. </p><p> </p><p> <strong><i> Materials and Methods </i></strong></p><p> In a cross-sectional study, 39 seropositive blood donors and 261 seronegeative ones in 2004 were studied. HBsAg was examined via ELISA. Standard hepatitis B risk factor questionnaires were completed for all of the samples and the prevalence of each risk factor in case and control subjects was compared through X<sup>2</sup> test, using SPSS-10 software with p<0.05. </p><p> <strong><i> </i></strong></p><p> <strong><i>Results</i></strong></p><p> This study indicated that the history of surgeries, use of shared razors, jaundice of mother, presence of HBsAg<sup>+</sup> patients and drug addicts in families were statistically significant in the two groups. There was not any hemodialysis history, accident of needle sticks and needle sharing by drug addicts. </p><p> </p><p> <strong><i>Conclusions</i></strong></p><p> Since the relative frequency of surgery history in the two groups was significantly different, attending to hospital and operation room hygine seems to be very important. Moreover, since hepatitis B can be prevented with education and vaccination, the families of <br>HBsAg<sup>+</sup> patients and drug addicts should be encouraged to use education and vaccination. </p><p> Using the results of this study, it is necessary to pay attention to hospitals and raise people awareness about hepatitis B transmission routes and vaccination of high risk individuals.</p><p> <strong> </strong></p><p> <strong> </strong></p><p> <strong><i>Key words</i></strong><i>: </i>Hepatitis B, Risk factors, Blood donors, Isfahan </p>
