<p> <strong> </strong></p><p align="justify"> چکید ه </p><p align="justify"> <strong><i> سابقه و هدف </i></strong></p><p align="justify"> <strong> بیماران تالاسمی ماژور بهعلت دریافت طولانی مدت خون، در معرض آلودگی به هپاتیتهای ویروسی مخصوصاً </strong><strong>B </strong><strong>و </strong><strong>C </strong><strong>قرار دارند و امروز یکی از مهمترین </strong><strong></strong><strong>مسائل بهداشتی این بیماران کنترل آلودگی به این ویروسهاست. دراین مطالعه برآن شدیم که شیوع هپاتیتهای </strong><strong>B </strong><strong>و </strong><strong>C </strong><strong></strong><strong>و عوامل خطرساز آن را درگروهی از بیماران تالاسمی ماژور بررسی کنیم. </strong></p><p align="justify"> <strong><i> مواد وروشها </i></strong></p><p align="justify"> <strong> مطالعه بهصورت توصیفی بر روی 364 بیمار تالاسمی ماژور در شهرستان زاهدان انجام شد. ابتدا تمام بیماران مورد پیگیری قرار گرفتند و پرسشنامهای شامل اطلاعات دموگرافیک از جمله سن، جنس، قومیت، تعداد دفعات تزریق خون در سال، سن تشخیص بیماری، سابقه واکسیناسیون و... تکمیل شد. سپس نمونه خون بیماران، تحت بررسی مارکرهای سرمی </strong><strong>Anti-HBc </strong><strong>، </strong><strong>HBsAg </strong><strong>، </strong><strong>Anti-HCV </strong><strong>، </strong><strong>Anti-HBs </strong><strong>به روش الیزا قرار گرفت و نتایج الیزا مثبت </strong><strong>HBsAg </strong><strong>و </strong><strong>Anti-HCV </strong><strong>به ترتیب توسط تست نوترالیزیشن و </strong><strong>RIBA </strong><strong>مورد تأیید قرار گرفت. نتایج توسط نرمافزار </strong><strong>SPSS 10/10 </strong><strong>بررسی شد و از آماره </strong><strong>Chi-square </strong><strong>برای آنالیز آماری استفاده شد. </strong></p><p align="justify"> <strong><i> یافته ها </i></strong></p><p align="justify"> <strong> 364 بیمار تالاسمی مورد بررسی قرار گرفتند که 206 نفر (6/56%) مذکر و 158 نفر (4/43%) مؤنث بودند. میانگین سنی بیماران 17/5 </strong><strong>± </strong><strong></strong><strong>7/9 سال بود که 205 نفر (3/56%) آنها بلوچ بودند. از این تعداد 1 نفر </strong><strong>HBsAg </strong><strong>مثبت و 49 نفر </strong><strong>Anti-HCV </strong><strong>مثبت بودند. در نتیجه شیوع </strong><strong>HBsAg </strong><strong>مثبت 3/0% و شیوع </strong><strong>Anti-HCV </strong><strong>مثبت 5/13% بود. 321 نفر (2/88%) </strong><strong>Anti-HBs </strong><strong>مثبت و 88 نفر (2/24%) </strong><strong>Anti-HBc </strong><strong>مثبت بودند. در بررسی آماری انجام شده بین سن بیماران و </strong><strong>Anti-HCV </strong><strong>مثبت اختلاف آماری معنیداری وجود داشت بهطوریکه با افزایش سن، </strong><strong>Anti-HCV </strong><strong>مثبت افزایش مییافت </strong><strong>(P<0.005) </strong><strong>. همچنین بین تعداد دفعات تزریق خون در سال و </strong><strong>Anti-HCV </strong><strong>مثبت نیز اختلاف آماری معنیداری وجود داشت، بهطوریکه افرادی که بیشتر یا مساوی 20 بار در سال خون دریافت کرده بودند، 3/33% </strong><strong>Anti-HCV </strong><strong>مثبت بودند درحالیکه در سایرگروهها این میزان کمتر از 15% گزارش شد </strong><strong>(P=0.006) </strong><strong>. </strong><strong></strong></p><p align="justify"> <strong><i> نتیجه گیری </i></strong></p><p align="justify"> <strong> با ارزیابی این مطالعه به این نتیجه میرسیم که شیوع هپاتیت </strong><strong>C </strong><strong>در بیماران تالاسمی ماژور بسیار بالاست اما بهنظر میرسد طرح غربالگری اهداکنندگان بهطور مؤثری میتواند شیوع و بروز عفونت را کاهش دهد </strong><strong>. </strong></p><p align="justify"> <strong><i> کلمات کلیدی: </i></strong><strong></strong><strong></strong><strong>تالاسم </strong><strong>ی </strong><strong>ماژور، هپاتیت </strong><strong>B </strong><strong></strong><strong>، </strong><strong>هپاتیت </strong><strong>C </strong><strong></strong></p>
<p> <strong> Abstract </strong><strong></strong></p><p> <strong><i> </i></strong></p><p> <strong><i>Background and Objectives</i></strong></p><p> Patients with major b -thalassaemia have a potential risk for infection cause of hypertransfusion and today one of the most improtant health problems in these patients is prevention and control of these viruses. In this study, we determined prevalence of hepatitis B and C and risk factors in a group of patients with major b -thalassaemia</p><p> <strong><i> </i></strong></p><p> <strong><i>Materials and Methods</i></strong></p><p> This descriptive study was performed in Zahedan city in 1381 on 364 patients with major <br>b -thalassaemia. At first, a questionnaire containing some questions about demographic characteristics was filled for all patient, including: age, sex, frequency of blood transfusion in year, time of diagnosis, history of vaccination, then blood samples of patients were tested to detect serum markers including HBsAg, Anti-HBc, Anti-HCV, Anti-HBs with Eliza method. Positive EIA results of HBsAg and Anti-HCV were confirmed respectively with neutralization test and RIBA.</p><p> By using SPSS 10/10 software we analyzed our data the results were also analyzed by <br>Chi-square.</p><p> <strong><i> </i></strong></p><p> <strong><i>Results</i></strong></p><p> We studied 364 patients with major b -thalassaemia, 206 (56.6%) were male and 158 (43.4%) female. Average of patients age was 9.7 ± 5.17. 205 (56.3%) patients were Balooch. Out of them one case was HBsAg positive and 49 cases were Anti-HCV positive, therefore, prevalence of HBsAg and Anti-HCV positive were respectively 0.3% and 13.5%. 321 cases (88.2%) were Anti-HBs positive and 88 (24.2%) were Anti-HBc positive.</p><p> Statistical analysis showed that HCV infection was more prevalent among older patients.</p><p> Prevalence of Anti-HCV positive test in hypertransfusion patients (>20 times) was more (33.3%) compared with other age groups (<15%) (P=0.006).</p><p> <strong><i> </i></strong></p><p> <strong><i>Conclusions</i></strong></p><p> Our results showed that prevalence of hepatitis C in patients with major b -thalassaemia in Zahedan is very high, but it seems screening of blood donor effectively decreases prevalence and incidence of infection.</p><p> <strong> </strong></p><p> <strong> </strong></p><p> <strong><i>Key words:</i> <i /></strong>Major b -thalassaemia, Hepatitis B, Hepatitis C</p>
