fa مروری بر سه روش مورد استفاده برای تهیه فرآورده پلاکتی از خون کامل و روش آفرزیس An overview of three methods used to prepare the platelet components from whole blood and apheresis method انتقال خون Blood Transfusion مروري Review Article <div style="text-align: justify;">چکیده سابقه و هدف روشهای متفاوتی برای تهیه پلاکت وجود دارد. هر یک از این روشها برای مراکز انتقال خون، اهداکنندگان و بیماران میتوانند مزایا و محدودیتهایی داشته باشند. آگاهی از آنها برای تصمیمگیری در مورد انتخاب روش مناسب در مراکز انتقال خون لازم است. این مقاله به مرور روشهای تهیه پلاکت و مقایسه آنها می‌پردازد. مواد و روش‌ها در یک مطالعه مروری، مقاله‌های مرتبط با تهیه پلاکت جستجو شد و متون مقاله‌ها و مراجع شناسایی و استخراج گردید. کلید واژه‌های blood component ، platelet-rich plasma ، platelet concentrate ، buffy-coat method ، platelet storage ، buffy-coat pooled platelet ، plateletpheresis ، pathogen inactivation ، Overnight storage ، preparation concentrate ، Leukofiltered platelets در پایگاه‌های داده PubMed, Web Of Science, Scopus, Magiran, SID و موتور جستجوی Google و Google Scholar در بازه اردیبهشت تا خرداد 1401 جستجو شد. یافته‌ها مطالعه‌ها در مورد مقایسه روشهای تهیه پلاکت نتایج مختلفی داشتهاند. در روش بافی‌کوت در هنگام سانتریفیوژ، گلبول‌های قرمز به عنوان تکیه‌گاهی برای پلاکتها عمل می‌کنند، در حالی که در روش پلاسمای غنی از پلاکت، پلاکتها در معرض دیواره پلاستیکی کیسه قرار می‌گیرند و منجر به فعال شدن پلاکتها می‌شود. برخی یافتههای جدیدتر نشان داده‌اند کیفیت و اثر بخشی بالینی روش بافیکوت بهتر از روش پلاسمای غنی از پلاکت و معادل روش آفرزیس است. با این حال هنوز شواهد کافی در دسترس نیست. محدودیتهای تهیه پلاکت به روش آفرزیس شامل هزینه، محدودیت جذب اهداکنندگان و زمانبر بودن فرآیند باعث تمایل به سمت سایر روش‌ها شده است. به هر حال در مواردی مانند مقاومت پلاکتی، تهیه پلاکت همسان به روش آفرزیس ضروری است. نتیجه گیری برای بهبود کیفیت پلاکت‌ها و بهینه‌سازی درمان بیماران ترومبوسایتوپنیک تهیه پلاکتهای ادغام شده کم لکوسیت با توجه به چالشهای روش آفرزیس گزینه مناسبی است. با پیشرفت روش‌هایی مانند کاهش پاتوژن و مواد افزودنی پلاکتی سعی می‌شود ایمنی و عمر نگهداری این فرآورده بیشتر گردد.  </div> <div dir="rtl" style="text-align: left;">Abstract Background and Objectives There are different ways to prepare platelets. Every one of these methods can have advantages and limitations for blood transfusion centers, donors, and patients. Knowledge of them is necessary for choosing the proper technique in blood transfusion centers. This article reviewed the methods of preparing platelets and compared them. Materials and Methods Studies related to the preparation of platelet components, the texts of articles, and review references were extracted. PubMed, Web of Science, Scopus, Magiran, SID, Google, and Google Scholar search engines were used for keywords such as blood component, platelet-rich plasma, platelet concentrate, buffy-coat method, platelet storage, buffy-coat pooled platelet, plateletpheresis, pathogen inactivation, Overnight storage, preparation concentrate, Leukofiltered platelets from May to June 2022. Results Studies comparing platelet preparation methods have had different results. One of the advantages of Buffy–coat method may be less activation, as platelets are centrifuged against a cushion of RBCs rather than the plastic container during PRP-PC production. Some recent findings have shown that the quality and clinical effectiveness of the buffy-Coat technique is better than Platelets Rich plasma (PRP) and is equivalent to the apheresis method. However, sufficient evidence is not yet available. The limitations of platelet preparation by the apheresis method, including the cost, declining apheresis donor base, and the time-consuming process, have led to the tendency towards other techniques. However, in cases such as platelet refractoriness, it is necessary to prepare matched platelets using the apheresis method. Conclusions  To improve the quality of platelets and optimize the treatment of thrombocytopenic patients, it seems that the preparation of leuko-reduced pooled platelets is a suitable option considering the challenges of the apheresis method.
