fa شیوع آلوایمونیزاسیون در بیماران مبتلا به کم خونی داسی شکل استان خوزستان Prevalence of alloimmunization in patients with sickle cell anemia in Khuzestan province ايمونوهماتولوژي Imunohematology پژوهشي Research <div style="text-align: justify;">چکیده سابقه و هدف بیماری آنمی داسی شکل بعد از تالاسمی، دومین هموگلوبینوپاتی شایع با عوارضی مانند انسداد عروقی و هم‌چنین آنمی همولیتیک است. تزریق خون به عنوان یک درمان حمایتی برای این بیماران به شمار می‌رود. از این رو، یکی از چالش‌های مهم در این بیماران، آلوایمونیزاسیون گلبولهای قرمز در اثر تزریق خون است و میتوان از شناسایی واحد خون مناسب از نظر آنتی‌ژن منفی برای تزریق به بیماران به عنوان یکی از مشکلات اصلی انتقال خون در این بیماران یاد کرد. در این مطالعه، شیوع آلوایمونیزاسیون در بیماران داسی‌شکل استان خوزستان بررسی شد. مواد و روش‌ها در این مطالعه که از نوع مقطعی غیر مداخله‌ای (توصیفی) بود، پس از اخذ رضایت از 184 بیمار کمخونی داسیشکل و داسی- تالاسمی مراجعه‌کننده به بیمارستان شهید بقایی2 اهواز که با شیوه نمونه‌گیری غیرتصادفی انتخاب شدند، نمونه خون محیطی حاوی ضد انعقاد EDTA دریافت شد، میزان آلوایمونیزاسیون و شناسایی و تعیین آلوآنتی‌بادی به روش هماگلوتیناسیون تعیین شد. یافته‌ها میزان شیوع آلوایمونیزاسیون در این جمعیت ۱۳% بود. در 24 بیمار 12 نوع آلوآنتی‌بادی شناسایی شد. شایع‌ترین آلوآنتی‌بادی به ترتیب Anti-E (8=n ، 33%)،  Anti-c(5 n= ، 8/20%) و Anti-D (4= n ، 6/16%) و Anti- Kell (4 n= ، 6/16%) بود. نتیجه گیری شایع‌ترین آلوآنتی‌بادی در بیماران آنمی داسی ‌شکل استان خوزستان، علیه سیستم گروه خونی Rh و سپس Kell بود. بنابراین توصیه می‌شود به این افراد خون سازگار از نظر آنتی‌ژن‌های Rh (D,C,c,E) و Kell تزریق شود.  </div> <div dir="rtl" style="text-align: left;">Abstract Background and Objectives Sickle Cell Disease is the second most common hemoglobinopathy after thalassemia and is characterized by vascular occlusion and hemolytic anemia. Blood transfusion is considered as a supportive treatment for patients with sickle cell disease. The alloimmunization of red blood cells is a major challenge in these patients. Accordingly, supplying the appropriate  antigen-negative blood units is a major problem in blood transfusion. The aim of this study was to evaluate the alloimmunization rates among individuals with Sickle Cell Disease in Khuzestan province. Materials and Methods In this descriptive cross-sectional study, a total of 184 patients with sickle cell anemia and sickle-thalassemia were referred to the Shahid Beqaei hospital in Ahvaz. 5-10 mL of peripheral blood containing the anticoagulant EDTA was collected by non-random sampling procedure. The prevalence of phenotypes and the rate of alloimmunization were determined by the hemagglutination technique. Results The prevalence of alloimmunization in this population was 13.0%. In 24 patients, twelve types of alloantibodies were detected. The most common alloantibodies were Anti-E (33.3%, n = 8), Anti-c (20.8%, n = 5), Anti-D (16.6%, n = 4), and Anti- Kell (16.6%, n = 4). Conclusions  Antibody against Rh blood group system and anti-Kell were the most prevalent alloantibodies among sickle cell anemia patients in the province of Khuzestan.
