fa حساسیت، ویژگی و ارزش اخباری مثبت در کیت‌های غربالگری ویروسی اهداکنندگان سازمان انتقال خون ایران در دو گروه پرخطر و کم خطر در سال‌های 1397 تا نیمه اول سال 1398 Evaluation of sensitivity, specificity and positive predictive value in viral screening kits of donors of Iran Blood Transfusion Organization in two high-risk and low-risk groups in 1397 to the first half of 1398 ويروس شناسي Virology پژوهشي Research <div style="text-align: justify;">چکیده سابقه و هدف حساسیت، ویژگی و ارزش اخباری کیت‌های شناسایی عفونت‌های منتقله از راه خون، از شاخص‌های مهم ارزیابی و کاربری کیت است. با توجه به استفاده وسیع از کیت الایزا در شناسایی عفونت‌ها، هدف این مطالعه بررسی کیت‌های فوق بود. مواد و روش‌ها در این مطالعه توصیفی، حساسیت کیت‌ها با استفاده از پانل‌‌های معتبر کمپانی بوستون بیومدیکا و نمونه‌های مثبت قطعی بیماران ایرانی و ویژگی و ارزش اخباری مثبت کیت‌ها در دو گروه کم خطر(1646371) و پرخطر (2525) محاسبه گردید. نتایج با آزمون‌های آماری توصیفی26 SPSS وOR  تحلیل شده و 05/0 p< معنادار در نظرگرفته شد. یافته‌ها در ارزیابی کیت‌های Monalisa HBsAg ULTRA بیوراد و Genscreen ULTRA HIV Ag/Ab بیوراد حساسیت حتی بیش از کیت‌های مرجع(Siemens ، 6/0 Enzygnost HBsAg و HIV Ag/Ab بیومریوکس) و در مورد کیت Monalisa Anti-HCV PLUS Version 3 بیوراد حساسیت معادل 6/98% کیت مرجع(Ortho HCV 3.0 Enhanced SAVE) گزارش گردید. نتایج ارزیابی ویژگی در گروه پرخطر در سه کیت HBs Ag ، Anti HCV و HIV Ag/Ab به ترتیب 56/99% ، 100% و 80/99% و ارزش اخباری مثبت در گروه کم خطر به ترتیب 04/41% ، 30/31% و 07/6% و در گروه پر خطر به ترتیب 5/72%، 100% و 5/37% گزارش شد. نتیجه گیری نتایج پژوهش حاضر علاوه بر مناسب بودن حساسیت و ویژگی کیت‌های مورد بررسی، تفاوت چشمگیر ارزش اخباری مثبت در جوامع با شیوع کم خطر و پرخطر ویروسی را نشان داد و بر اهمیت توجه به شیوع به منظور انتخاب کیت و روش مورد استفاده جهت غربالگری ویروسی هر چه بیشتر تأکید نمود.</div> <div dir="rtl" style="text-align: left;">Abstract Background and Objectives Assessing the sensitivity, <a name="_Hlk75531029">specificity </a>and positive predictive value (PPV) of blood-borne infections detection kits are important indicators in the evaluation and use of the kit.   Materials and Methods The present study is a descriptive study. The sensitivity of the kit was calculated using the panels of Boston Biomedica Company, definite positive samples of Iranian patients, and the specificity and PPV of the kits in two groups of low-risk )1646371) and high-risk (2525). The results were analyzed with descriptive statistics, SPSS 26, and OR statistical test. (p < 0.05)   Results In evaluating Monalisa HBs Ag ULTRA, Bio-Rad, and Genscreen ULTRA HIV Ag/Ab kits, Bio-Rad is even more sensitive than reference kits (Enzygnost HBsAg 6.0, Siemens, HIV Ag/Ab Biomerieux) and in the case of Monalisa Anti-HCV kit PLUS Version 3, Bio-Rad sensitivity was equal to 98.6% of the reference kit (Ortho HCV 3.0 Enhanced SAVE). The results of the specificity evaluation in the high-risk group by three kits of HBs Ag, Anti HCV, and HIV Ag/Ab were 99.56%, 100%, and 99.80%, respectively. The results of the evaluation of PPV in the same three kits in the low-risk group were 41.4%, 31.30%, and 6.07% and in the high-risk group they were reported to be 72.5%, 100% and 37.5%, respectively.   Conclusions  The results of the present study, in addition to the appropriateness of sensitivity and specificity of the studied kits, showed a significant difference in PPV in communities with different viral prevalence and emphasized the importance of paying attention to prevalence in order to select kits and methods used for viral screening or diagnosis.</div> ارزش اخباری تست, حساسیت, ویژگی Predictive Value of Tests, Sensitivity, Specificity 149 160 http://bloodjournal.ir/browse.php?a_code=A-10-1462-1&slc_lang=fa&sid=1 ُS.
